Er is geen duidelijkheid over klinisch-pathologische implicaties van tumor spread through air spaces (STAS) na sublobaire resectie voor stadium IA part-solid longadenocarcinoom. Een retrospectieve studie in het Bundang Ziekenhuis van Seoel Nationale Universiteit (Zuid-Korea) heeft de prognostische impact van STAS in deze setting onderzocht. Prof. Sanghoon Jheon en collega’s publiceren de studie in Lung Cancer.¹

Tussen februari 2009 en januari 2017 ondergingen in het ziekenhuis 115 patiënten met stadium IA part-solid adenocarcinoom sublobaire resectie. Twintig van deze patiënten (17,4%) hadden STAS. In multivariate analyse was STAS onafhankelijk geassocieerd met verhoogd risico van recidief (HR 9,4; p=0,002). Vijf-jaars freedom from recurrence werd gezien in 62,4% van de STAS-patiënten versus 97,9% van de niet-STAS patiënten (p<0,001), en de vijf-jaars ziektevrije overlevingspercentages waren 58,5% versus 97,9% (p<0,001). Aanwezigheid van STAS was geassocieerd met hogere leeftijd (p=0,030), mannelijk geslacht (p=0,023), predominant acinair histologisch patroon (p=0,004), micropapillair patroon (p<0,001), lymfovasculaire invasie (p<0,001), en grotere invasieve component (p<0,001).

De onderzoekers concluderen dat STAS een belangrijke prognostische factor kan zijn in patiënten die sublobaire resectie ondergaan voor stadium IA part-solid longadenocarcinoom.
