De
internationale Berlin-Frankfurt-Münster (BFM) AML studiegroep heeft een studie
uitgevoerd van de prognostische relevantie van twee verschillende
t(16;21)-translocaties in pediatrisch AML. Sanne Noort (Erasmus MC) en
collega’s publiceren de studie online in Blood.1 De translocaties in kwestie zijn t(16;21)(p11;q22),
resulterend in het FUS-ERG fusiegen, en t(16;21)(q24;q22) resulterend in
RUNX1-CBFA2T3.
De studie
includeerde 54 pediatrische AML-patiënten met t(16;21) translocaties, met het
AML-BFM cohort gediagnostiseerd tussen 1997 en 2013 als referentiecohort. De
groep van 23 patiënten met RUNX1-CBFA2T3 had significant lagere mediane white blood count (12,5 x 109;
p=0,03) dan het referentiecohort. De groep van 31 patiënten met FUS-ERG
rearranged AML had geen predominant FAB-type, terwijl 76% van de groep met
RUNX1-CBFA2T3 een M1/M2 FAB-type had, significant verschillend van het
referentiecohort (p=0,004). De vier-jaars gebeurtenisvrije overleving van
patiënten met FUS-ERG was 7%, significant lager dan de 51% in het
referentiecohort (p<0,001). De vier-jaars EFS van RUNX1-CBFA2T3 was 77%, dus
hoger dan het referentiecohort (p=0,06). De cumulatieve incidentie van relapse
was 74% in FUS-ERG, 0% in RUNX1-CBFA2T3, en 32% in het referentiecohort.
De
onderzoekers concluderen dat de studie twee klinisch relevante onderscheiden
subtypen van pediatrisch AML identificeerde. Patiënten met
RUNX1-CBFA2T3-rearranged AML kunnen profiteren van stratificatie in de
standaard-risicobehandeling, en FUS-ERG rearranged AML dient te worden gezien
als hoog-risico ziekte.

lower and the translocation resulting in RUNX1-CBFA2T3 was associated with a
higher 4-years EFS compared to the AML-BFM reference cohort.
