De multicenter fase 3-studie FIL-DLCL04 in Italië vergeleek verschillende
behandelingen voor nieuw-gediagnostiseerd diffuus grootcellig B-cel lymfoom in
jonge patiënten (leeftijd 18 tot en met 65 jaar). Een secundaire analyse van de
studie heeft de prognostische impact van TP53-mutaties
in DLBCL geïnventariseerd. Dr. Annalisa Chiappella (Istituto Nazionale dei
Tumori, Milaan) en collega’s publiceren de analyse in het British Journal of
Haematology.1
Van de 412
patiënten in de studie includeerde de analyse 125 patiënten met beschikbare
tumorblokken. TP53-mutaties werden
gezien in 11 van 125 patiënten (9%). Zestig van 125 patiënten kregen upfront hoge-dosering chemo-immuuntherapie.
De mediane follow-up was 72 maanden. Het vijf-jaars faalvrije overlevings
(FFS)-percentage was 24% in de groep met TP53-mutaties versus 72% in de groep
zonder TP53-mutaties (HR 2,28; p=0,086); het vijf-jaars overall survival percentage was 34% versus 83% (HR 4,05; p=0,011).
De
onderzoekers concluderen dat TP53-mutaties
in nieuw-gediagnostiseerd DLBCL in jonge patiënten geassocieerd waren met
slechte prognose, ongeacht behandeling of biologische kenmerken.
