De Breast Cancer Index (BCI), gebaseerd op
expressie van elf genen, is ontwikkeld om het risico van recidief van
vroeg-stadium HR-positief mammacarcinoom te schatten. Een retrospectieve multicenterstudie in
de Verenigde Staten heeft de waarde van BCI voor het schatten van risico van
afstandsrecidief (DR) in HR-positief invasief lobulair carcinoom (ILC) geïnventariseerd.
Dr. Otto Metzger (Dana-Farber Cancer Institute, Boston MA) en collega’s
publiceren de studie in Clinical Cancer Research.1
De studie
includeerde 307 patiënten (99% ER-positief, 53% T1, 42% N+, 70% graad II).
BCI-genexpressieanalyse werd uitgevoerd geblindeerd voor klinische uitkomsten.
Het primaire eindpunt was tijd tot DR. BCI-low risk en BCI-intermediate risk
patiënten hadden overeenkomende tien-jaars DR-percentages (7,6% respectievelijk
8,0%), vergeleken met 27,0% voor BCI-high risk patiënten. BCI-high risk was een
significante onafhankelijke prognostische factor voor tien-jaars DR (HR 4,09; p=0,0001),
alsmede voor vroeg DR tijdens de eerste vijf jaar (HR 8,19; p=0,0042) als voor laat
DR tijdens jaren zes tot en met tien (HR 3,04; p=0,0024).In multivariate
analyse was BCI-high risk de enige statistisch significante prognostische
factor voor DR (HR 3,49; p=0,0150).
De
onderzoekers concluderen dat in patiënten met HR-positief vroeg-stadium ILC,
BCI een onafhankelijke prognostische factor was voor tien-jaars DR, vroeg DR,
en laat DR. BCI voegde prognostische waarde toe boven die van klinisch-pathologische
kenmerken.

independent prognostic factor for 0-5 year and 5-10 year distant recurrence of early-stage
