Er is geen duidelijkheid over de prognostische waarde van de neutrofiel/lymfocyt-ratio in perifeer bloed van patiënten met gevorderd pulmonair grootcellig neuro-endocrien carcinoom (LCNEC). Een retrospectieve studie in het ziekenhuis van het Japanse Nationaal Maligniteitencentrum (Tokio) heeft deze waarde onderzocht. Dr. Masayuki Shirasawa en collega’s publiceren de studie vandaag in het British Journal of Cancer.¹

De studie includeerde 63 patiënten die chemotherapie kregen voor pulmonair LCNEC. De overall survival in de groep patiënten met NLR lager dan 5 was significant langer dan die in de groep patiënten met NLR 5 of hoger (14,9 versus 5,2 maanden; p<0,001). In multivariate analyse was NLR 5 of hoger voorspellend voor slechte OS (HR 3,43; p<0,001). De NLR was invers gecorreleerd met gehalten van tumor (r= -0,648; p=0,005) en stroma (r= -0,490; p=0,046) CD8-positieve tumorinfiltrerende lymfocyten.

De onderzoekers concluderen dat hoge NLR geassocieerd was met slechte prognose van gevorderd pulmonair LCNEC. NLR was ook een indicator van de immuunstatus van de tumor.

Summary: A retrospective study at the Japanese National Cancer Center Hospital (Tokyo) found that a high neutrophyl-to-lymphocyte ratio in peripheral blood of patients with advanced-stage pulmonary large cell neuroendocrine carcinoma was associated with poor prognosis. The study revealed that the NLR can at least in part reflect the tumor microenvironment, suggesting that the NLR plays an important role not only as a clinical outcome predictor but also as a tumor immune status indicator.
