Het is niet duidelijk of vroege introductie van chemotherapie (CT) voor HR-positief HER2-negatief metastatisch mammacarcinoom (HR+ HER2- MBC) resistentie tegen endocriene therapie (ET) induceert. Een retrospectieve multicenterstudie in Frankrijk heeft progressievrije overleving op eerste ET voor of na CT geïnventariseerd. In de ESME-database van achttien Franse centra identificeerden de onderzoekers 6293 patiënten met HR+HER2- MBC die tussen begin 2008 en eind 2014 ET als eerstelijns behandeling kregen (n=3832; 60,9%) of als tweedelijns behandeling na eerstelijns CT (n=2461; 39,1%). De mediane PFS van de eerste groep was 12,4 maanden (95%-bti 11,9-13,1) en de mediane PFS van de tweede groep was 12,6 maanden (12,1-13,4). Het verschil was statistisch niet significant (p=0,13).

De onderzoekers concluderen dat de studie geen aanwijzingen heeft laten zien voor inductie van endocriene resistentie door CT.

Summary: A retrospective multicenter study in France found that among patients with HR-positive HER2-negative metastatic breast cancer, progression-free survival under first endocrine therapy was identical whether prescribed before or after chemotherapy. This observation suggests that chemotherapy does not promote endocrine resistance.
