Patiënten met
vroeg stadium natural killer/T-cel lymfoom (NK/TCL) krijgen gewoonlijk een
combinatie van chemotherapie en radiotherapie, maar de optimale behandeling is
nog niet vastgesteld. Een prospectieve multicenterstudie in China heeft de
combinatie van GELAD-chemotherapie (gemcitabine, etoposide , pegaspargase, en
dexamethason) met sandwich radiotherapie voor nieuw-gediagnostiseerd stadium IE-IIE
NK/TCL geëvalueerd. Dr. Rong Tao (Jiatong Universiteit, Shanghai) en collega’s
publiceren de studie in het British Journal of
Haematology.1
De studie
includeerde 52 patiënten die eerst twee cycli GELAD kregen, gevolgd door IMRT
(50-56 Gy in 25-28 fracties), en tenslotte nog twee cycli GELAD. De behandeling
resulteerd in respons in 49 patiënten (94,2%) en complete respons in 48
patiënten (92,3%). De mediane follow-up was 32 maanden. De vier-jaars
progressievrije overleving en overall
survival percentages waren 90,4% (95%-bti 78,4-95,9) respectievelijk 94,2%
(83,2-93,1). De meest-gerapporteerde adverse events hingen samen met
pegaspargase. De hematologische toxiciteiten waren mild. Graad 3-4 neutropenie
werd gezien in 15,4% van de patiënten.
De
onderzoekers concluderen dat GELAD-chemotherapie met sandwich IMRT aanzienlijke
activiteit had en goed verdragen werd in patiënten met nieuw-gediagnostiseerd vroeg-stadium
NK/TCL.

sandwiched IMRT for newly diagnosed early stage NK/TCL. Response was seen in
94.2% of patients and complete response in 92.3%. The four-year progression-free
and overall survival rates were 90.4% (95% CI 78.4-95.9) and 94.2% (83.2-93.1),
respectively. Grade 3-4 neutropenia was seen in 15.4% of patients.
