De rol van autologe stamceltransplantatie (autoSCT) in eerste complete remissie (CR1) van perifeer T-cel lymfomen (PTCLs) is niet goed gedefinieerd. Een analyse binnen de prospectieve COMPLETE-studie (Comprehensive Oncology Measures for Peripheral T-cell Lymphoma Treatment) heeft de waarde onderzocht van autoSCT in patiënten met nodaal PTCL in CR1. Onder de COMPLETE-patiënten waren 119 met nodaal PTCL die CR1 bereikten. Van deze patiënten ondergingen 36 consoliderende autoSCT en 83 niet. Na mediaan 2,8 jaar follow-up bedroeg was de mediane overall survival niet bereikt in de autoSCT-groep versus 57,6 maanden in de groep zonder autoSCT (p=0,06). AutoSCT was geassocieerd met superieure overleving onder patiënten met gevorderd-stadium ziekte of intermediaire en hoge IPI-scores. Subgroepanalyses lieten zien dat autoSCT geassocieerd was met significant betere OS en PFS in de groep patiënten met angioimmunoblastisch T-cel lymfoom, maar niet in patiënten met andere PTCL-subtypen. In multivariate analyse was autoSCT onafhankelijk geassocieerd met betere OS (HR 0,37; 95%-bti 0,15-0,89).

De onderzoekers concluderen dat autoSCT voor nodaal PTCL in CR1 in specifieke klinische scenario’s overlevingsprofijt kan bieden.
