Maligne pleuraal mesothelioom (MPM) is een agressieve maligniteit met slechte prognose en beperkte behandelopties. Een multicenter fase 3-studie in Italië heeft de waarde onderzocht van radicale hemithorax-radiotherapie (RHR) voor MPM. Eerste auteur van de studie, die vandaag wordt gepresenteerd op het ESTRO38-congres in Milaan, is dr. Emilio Minatel (Centro di Riferimento Oncologico, Aviano).¹

De studie includeerde 108 patiënten die niet-radicale longsparende chirurgie en chemotherapie hadden ondergaan voor MPM. De patiënten werden 1:1 gerandomiseerd naar radicale radiotherapie naar de hemithorax exclusief de long (50 Gy in 25 fracties plus simultane boost van 60 Gy naar gross disease) of palliatieve radiotherapie (PR, 20-30 Gy in 5-10 fracties) naar het thoracotomielittekenweefsel of alleen de tumor. Het eindpunt van de studie was overall survival. De mediane follow-up was 12,3 maanden. Na 64 OS-gebeurtenissen was de twee-jaars OS 58% in de RHR-arm versus 28% in de PR-arm (HR 0,46; p=0,003). Eén patiënt in de RHR-arm had graad 5-pneumonitis; zeventien patiënten hadden graad 3 toxiciteiten. In vier patiënten werd laat respiratoir falen gezien.

De onderzoekers concluderen dat RHR de overleving verbeterde van MPM-patiënten na niet-radicale chirurgie en chemotherapie, met acceptabele toxiciteit.

The two-years OS rate was 58% in the radical arm versus 28% in the palliative arm. The toxicity of radical radiotherapy was acceptable.
