Preoperatieve
radiotherapie resulteert in slechts een minderheid van de patiënten met
wekedelensarcoom (STS) in pathologische complete respons. NBTXR3 is een first-in-class hafniumoxide nanoparticle
met fysisch-chemische eigenschappen die het mogelijk maken tumorcellen binnen
te dringen en schade door radiotherapie te vergroten. De Franse multicenter
fase 2-3 studie Act.In.Sarc evalueerde NBTXR3 plus radiotherapie versus alleen
radiotherapie als preoperatieve behandeling voor lokaal-gevorderd STS. Dr.

De studie
includeerde 176 evalueerbare volwassen patiënten die preoperatieve
radiotherapie nodig hadden voor LA-STS van romp of extremiteit, alle
histologische graden, performance status 0, 1 of 2, en een levensverwachting
van tenminste zes maanden. Ze werden 1:1 gerandomiseerd naar intratumoraal
toegediend NBTXR3 plus RT (50 Gy in 25 fracties; n=87) of alleen RT (n=89),
gevolgd door chirurgie. Randomisatie was gestratificeerd naar histologisch
subtype (myxoïd liposarcoom versus andere histologie).
Het primaire
eindpunt was pathologisch complete respons. pCR werd gezien in veertien
patiënten (16%) in de NBTXR3-groep versus acht patiënten (9%) in de alleen-RT
groep. De meest-gerapporteerde graad 3 of 4 adverse
events gerelateerd aan NBTXR3-toediening waren injectieplaatspijn (4% van
de patiënten) en hypotensie (4%); graad 3 of 4 radiotherapie-gerelateerde AE
was stralingsschade van de huid (6% in de NBTXR3-groep versus 4% in de
alleen-RT groep. Serious adverse events
werden gezien in 39% versus 30%.
De
onderzoekers concluderen dat de studie werkzaamheid van deze nieuwe klasse
radioenhancer heeft gedemonstreerd.

soft-tissue sarcoma. A pathological complete response was noted in 16% of
patients in de NBTXR3 group versus 8% of patients in the radiotherapy alone
group (p=0,044). Serious adverse events were observed in 39% and 30%,
respectively.
