In vroege-fase studies is palbociclib monotherapie geëvalueerd voor goed-gedifferentieerd liposarcoom (WDLPS) en gedediferentieerd liposarcoom (DDLPS), met gerapporteerde mediane progressievrije overleving van 18 weken. Een retrospectieve studie van MD Anderson Cancer Center heeft real-world uitkomsten van palbociclib voor gevorderd WDLPS en DDLPS geïnventariseerd. Dr. Emily Keung en collega’s publiceren de studie in het International Journal of Cancer.¹

De studie includeerde achtereenvolgende patiënten met retroperitoneaal WDLPS (n=14) of DDLPS (n=47) die tussen begin januari 2016 en eind februari 2021 in MDACC palbociclib monotherapie kregen. Op het moment van start van palbociclib was de mediane leeftijd 64 jaar (IQR 56-72). Het mediane aantal eerdere systemische behandelingen was 0 (IQR 0-0) in het WDLPS-cohort en 2 (IQR 0-4) in het DDLPS-cohort, en het mediane aantal eerdere operaties was 2 in beide cohorten. De mediane PFS was 9,2 maanden (IQR 3,9-21,9) in het WDLPS-cohort en 2,6 maanden (IQR 2,0-6,1) in het DDLPS-cohort. Twaalf patiënten ondergingen uiteindelijk chirurgische resectie, met macroscopisch complete (R0/R1) resectie in zes van twaalf patiënten, onder wie slechts één patiënt na resectie recidief had (mediane follow-up 7,5 maanden). Alle patiënten met macroscopisch incomplete resectie hadden progressie, met mediane tijd tot progressie 3,3 maanden (IQR 2,3-4,4). Chirurgie na palbociclib-behandeling was niet geassocieerd met betere overall survival.

De onderzoekers concluderen dat werkzaamheid van palbociclib monotherapie voor gevorderd WDLPS en DDLPS teleurstellend was, zonder duidelijk profijt van de behandeling en lange-termijn tumorcontrole in slechts weinig patiënten.
