In verscheidene typen solide tumoren is synergie gezien tussen antitumorwerkzaamheid van immuuntherapie en chemotherapie. Deze combinatiestrategie is nog niet onderzocht in patiënten met gestationele trofoblastische neoplasie (GTN). Een retrospectieve studie in drie centra in China heeft anti-PD-1 therapie met of zonder chemotherapie voor hoog-risico chemorefractair of recidiverend GTN geëvalueerd. Prof. Yang Xiang (Peking Union Medical College, Beijing) en collega’s publiceren de studie in eClinicalMedicine.¹

Tussen begin augustus 2018 en eind maart 2022 werden in de drie centra 66 patiënten met hoog-risico chemorafractair of recidiverend GTN behandeld. Vijfendertig patiënten kregen alleen anti-PD-1 therapie en 31 patiënten kregen anti-PD-1 therapie plus chemotherapie. De objective response rate was 62,9% (22 van 35 patiënten) respectievelijk 96,8% (30 van 31 patiënten; p<0,001). De progressievrije overleving was significant langer in de combinatietherapiegroep dan in de monotherapiegroep (HR 0,06; p<0,001) en de overall survival (data nog niet matuur) was niet-significant langer in de combinatietherapiegroep (HR 0,50; p=0,499). Onder de patiënten die geen respons hadden in de anti-PD-1 monotherapiegroep hadden 11 van 13 (84,6%) alsnog respons op salvage chemotherapie. Treatment-related adverse events werden gezien in 80,6% versus 77,1% van de patiënten in de combinatietherapiegroep respectievelijk de monotherapiegroep (p=0,729)

De onderzoekers concluderen dat toevoegen van chemotherapie aan anti-PD-1 therapie voor chemofractair of recidiverend GTN resulteerde in verhoging van de respons en verlenging van de progressievrije overleving. De meerderheid van de patiënten zonder respons op anti-PD-1 therapie hadden respons op salvage chemotherapie.
