Richtlijnen
wijzen leverresectie (LR) voor multinodulair levercelcarcinoom (HCC) af. Een
retrospectieve cohortstudie in Italië heeft LR voor HCC met twee of drie noduli
die ieder niet groter waren dan 3 cm vergeleken met percutane
radiofrequentie-ablatie (PRFA) en transarteriële chemoëmbolisatie (TACE) in
patiënten die niet in aanmerking kwamen voor levertransplantatie. Dr.
Alessandro Vitale (Universiteit van Padua) en collega’s publiceren de studie in JAMA Surgery.1
De studie
includeerde 720 patiënten met vroeg-stadium multinodulair HCC (75% mannen; 49%
ouder dan 70 jaar). De LR-groep telde 296 patiënten, de PRFA-groep 240, en de
TACE-groep 184. Een matching-adjusted
indirect comparison (MAIC)-methode werd toegepast om potentiële confounding
te vermijden. Het vijf-jaars overlevingspercentage was 56,4% in de LR-groep,
vergeleken met 39,9% in de PRFA-groep en 29,9% in de TACE-groep. Multivariate
analyse liet zien dat de overleving na LR significant beter was dan na PRFA (HR
1,41; p=0,01) en na TACE (HR 1,86; p=0,001).
De
onderzoekers concluderen dat onder patiënten met vroeg-stadium multinodulair
HCC die niet in aanmerking kwamen voor transplantatie, LR resulteerde in betere
overleving dan PRFA of TACE.
