Obesitas is
een onafhankelijke risicofactor voor slechtere uitkomsten in verscheidene
chirurgische settings. Het is niet duidelijk of obesitas ook een prognostische
factor is voor postoperatieve morbiditeit en mortaliteit van patiënten die
chirurgie ondergaan voor colorectaalcarcinoom (CRC). Een retrospectieve analyse
van de US National Inpatient Sample
database heeft onderzocht wat de impact is van obesitas op korte-termijn
postoperatieve uitkomsten in CRC-patiënten die laparoscopische chirurgie
ondergaan. Prof. Ko-Chao Lee (Chang Gung Universiteit, Kaohsiung, Taiwan) en
collega’s publiceren de analyse in Supportive Care in Cancer.1
De analyse
includeerde 23.898 patiënten, onder wie 11,8% met obesitas (BMI 30 kg/m2
of hoger). Obesitas vergeleken met geen obesitas was geassocieerd met
verdubbeld risico van diepe veneuze trombose/pulmonaire embolie (DVT/PE; aOR
2,12; 95%-bti 1,32-3,41). Morbide obesitas (BMI 40 kg/m2 of hoger)
was geassocieerd met verhoogd risico van in-hospital
mortaliteit (aOR 2,06; 95%-bti 1,11-3,83), acute nierschade (1,78; 1,34-2,36),
DVT/PE (2,88; 1,70-4,88), en langer verblijf in het ziekenhuis (1,21;
1,02-1,43).
De
onderzoekers concluderen dat obesitas, en met name morbide obesitas,
geassocieerd was met slechtere postoperatieve uitkomsten onder patiënten die
laparoscopische chirurgie ondergingen voor CRC.

surgery for CRC, morbid obesity was strongly associated with worse
postoperative outcomes, including increased in-hospital mortality,
postoperative acute kidney injury, deep vein thrombosis/pulmonary embolism, and
extended hospital stay. Non-morbid obesity was associated with more DVT/PE.
