In eerdere
studies is gezien dat zowel immuuncheckpointremmers (ICIs) als anlotinib
werkzaamheid hebben in het centraal zenuwstelsel (CNS). Een retrospectieve
multicenterstudie in China heeft de combinatie van ICIs plus anlotinib voor
kleincellig longcarcinoom (SCLC) met hersenmetastasen (BMs) geëvalueerd. Dr.
Zhaoqin Huang (Shandong Universiteit) en collega’s publiceren de studie in het Journal of Neuro-Oncology.1
De studie
includeerde 66 patiënten met BMs van SCLC. De overall response rate op de combinatie van ICIs plus anlotinib was
29,2% (RECIST 1.1). De mediane intracraniële-progressievrije overleving was 9,0
maanden (95%-bti 6,5-11,5) en de mediane overall
survival was 13,4 maanden (10,7-20,5). Factoren die een ongunstige
associatie met OS hadden waren hoge ziektelast (HR 4,83; p<0,001), meerdere
BMs (2,71; p=0,036), en drie of meer eerdere lijnen behandeling (2,56;
p=0,023). De incidentie van treatment-related
adverse events was 75,8%; graad 3 of 4 TRAEs werden gerapporteerd voor
19,7%.
De
onderzoekers concluderen dat de combinatie van ICIs met anlotinib sterke CNS-werkzaamheid
en verdraagbare toxiciteit had onder patiënten met BMs van SCLC.
