Patiënten
met thymuscarcinoom (TC) hebben een slechte prognose. Een multicenter
retrospectieve studie in China heeft eerstelijns chemotherapie met of zonder
immuuncheckpointremmers (ICIs) voor niet-resectabel TC geëvalueerd. Prof. Likun
Chen (Sun Yat-sen Universiteit, Guangzhou) en collega’s publiceren de studie in het International
Journal of Cancer.1
De studie
includeerde 93 patiënten met niet-eerder behandeld niet-resectabel TC. Onder
deze patiënten kregen 54 patiënten chemotherapie en 39 chemotherapie plus ICIs.
De objective response rate was 50% in de chemotherapiegroep en 76,9% in de
chemotherapie plus ICI-groep. De mediane
progressievrije overleving was 8,8 maanden in de chemotherapiegroep en 34,9
maanden in de chemotherapie plus ICI-groep (p<0,001), en de mediane overall survival was 41,5 maanden
respectievelijk niet bereikt (p=0,025). Immuun-gerelateerde adverse events werden gezien in 17
patiënten in de chemotherapie plus ICI-groep, onder wie 15 met graad 1 of 2
irAEs.
De
onderzoekers concluderen dat onder patiënten met niet-resectabel TC,
eerstelijns chemotherapie plus ICIs superieure werkzaamheid heeft vergeleken
met alleen chemotherapie, en dat de adverse events manageable zijn.
