Er is geen consensus over de optimale primaire therapie voor gelokaliseerd myxofibrosarcoom (MFS). Na primaire resectie wordt in een hoog percentage van de patiënten lokaal recidief gezien, terwijl de ziekte vaak ook refractair is tegen niet-chirurgische behandeling. Een retrospectieve studie van het sarcoomcentrum Helios Klinikum Berlin-Buch heeft uitkomsten met multimodale behandeling voor MFS geïnventariseerd. Dr. Saeed Ghani en collega’s publiceren de studie in The Oncologist.¹


De studie includeerde 134 patiënten met gelokaliseerd MFS, die in Berlijn multimodale therapie kregen met combinaties van chirurgie, bestraling, chemotherapie, hyperthermie, en isolated limb perfusion. Het vijf-jaars overall survival percentage was 74,9% terwijl het vijf-jaars lokaal-recidiefvrije overlevingspercentage en het vijf-jaars afstandsmetastasevrije-overlevingspercentage 66,1% respectievelijk 80,8% bedroegen. Factoren met ongunstige impact op OS waren hogere leeftijd, tumorgrootte cT2 of hoger, niet-extremiteit locatie, en afstandsmetastase. Factoren met gunstige impact op lokaal recidief waren incisiebiopsie, wide local excision, negatieve marges, en radiotherapie. Tumorgrootte cT3 of hoger was een ongunstige voorspellende factor voor afstandsmetastase. Adjuvante bestraling had een gunstige impact voor lokaal-recidiefvrije overleving in alle gelokaliseerde tumorstadia, zelfs in cT1-tumoren. Chemotherapie had geen significante impact op afstandsmetastase-vrije overleving.

De onderzoekers concluderen dat gelokaliseerd MFS dient te worden beschouwd als en chemotherapie-refractaire entiteit, met combinatie van wide local excisie en radiotherapie als primaire behandeling.
