Risico van bleomycine-gerelateerde pulmonaire toxiciteit na post-chemotherapie retroperitoneale lymfeklierdissectie in patiënten met gunstig-risico kiemceltumoren


Dr. Clint CaryDrie cycli bleomycine, etoposide, en cisplatine (BEP x 3) of vier cycli etoposide en cisplatine (EP x 4) zijn eerstelijns chemotherapieregimes voor mannen met gunstig-risico kiemceltumoren (GCTs). Dr. Clint Cary en zijn collega’s van Indiana University School of Medicine in Indianapolis hebben onderzocht of inclusie van bleomycine van invloed is op pulmonaire en operatieve morbiditeit na post-chemotherapie retroperitoneale lymfeklierdissectie (PC-RPLND). Ze publiceren de studie online in het Journal of Clinical Oncology.1

In de database van hun ziekenhuis identificeerden de onderzoekers 234 patiënten die tussen begin 2006 en eind 2016 BEP x 3 (n=191) of EP x 4 (n=43) inductiechemotherapie voor PC-RPLND kregen. Primaire uitkomsten van de studie waren pulmonaire toxiciteit (verlengde intubatie, reïntubatie, zuurstofsuppletie, verblijf op afdeling intensieve zorg) en operatieve morbiditeit (operatieduur, concomitante procedures, geschat bloedverlies).


Geen van de patiënten werd gereïntubeerd of ontslagen met zuurstof. Twee patiënten in elk van beide groepen moesten om niet-pulmonaire redenen worden opgenomen op de afdeling intensieve zorg. Patiënten in de BEP-groep hadden significant korter gebruik van zuurstofsuppletie dan patiënten in de EP-groep (gemiddeld 0,99 versus 1,63 dagen; p=0,005). Er waren geen significante verschillen in preoperatieve massa (p=0,42) of concomitante chirurgie (p=0,58). In de BEP-groep was de operatietijd significant korter (131 versus 170 minuten; p<0,01) en het geschatte bloedverlies significant minder (194 versus 226 ml; p<0,01) dan in de EP-groep. Duur van verblijf in het ziekenhuis was korter in de BEP-groep dan in de EP-groep (3,3 versus 3,9 dagen; p<0,01).

De onderzoekers concluderen dat in een moderne-chirurgiecohort inclusie van bleomycine in het inductiechemotherapieregime geen ongunstige invloed had op pulmonaire morbiditeit, operatietijd, of niet-pulmonaire postoperatieve complicaties na PC-RPLND voor gunstig-risico GCT.

Summary: A study at Indiana University School of Medicine showed that, in a modern surgical cohort, the inclusion of bleomycin in first-line chemotherapy for good-risk germ cell tumors does not seem to influence pulmonary morbidity, operative difficulaty, or nonpulmonary postoperative complications after postchemotherapy retroperitoneal lymph node dissection.
