PARP-remmer (PARPi)
onderhoudstherapie kan resulteren in verbetering van de uitkomsten na
eerstelijns behandeling voor gevorderd ovariumcarcinoom (aOC). De multicenter
retrospectieve GORILLA-3004 studie (‘gynecologic
oncology reseach investigators collaboration’) in Zuid-Korea heeft
risicofactoren voor falen van eerstelijns PARPi-onderhoudsbehandeling voor aOC geïnventariseerd.

De studie
includeerde 191 patiënten die PARPi-onderhoudsbehandeling kregen na primaire
behandeling voor stadium III of IV epitheliaal OC. In 63,4% van de patiënten
werden BRCA-mutaties gezien. De
mediane follow-up was 9,9 maanden. Tijdens de follow-up werd recidief gezien in
20,9% van de patiënten. Onder de patiënten met recidiverende ziekte had een
hoger percentage postoperatieve residuele tumor (60,5% versus 37,8%),
niet-hooggradig sereus carcinoom (HGSC; 15,0% versus 6,0%), neoadjuvante
chemotherapie (NACT; 55,0% versus 35,8%), en pre-PARPi serum CA-125 23,5 U/ml
of hoger (35,9% versus 15,2%) vergeleken met de patiënten zonder recidiverende
ziekte. In multivariate analyse waren pre-PARPi serum CA-125 23,5 U/ml of hoger
(HR 2,17; p=0,042), non-HGSC (3,28; p=0,021), NACT (2,11; p=0,037), en geen BRCA-muatie (2,23; p=0,023)
onafhankelijke risicofactoren voor falen van de onderhoudsbehandeling. Onder de
patiënten met BRCA-mutatie was hoog pre-PARPi
CA-125 niveau de enige risicofactor voor progressie.
De
onderzoekers concluderen dat hoge pre-PARPi serum CA-125 niveaus, non-HGSC
histologie, NACT, en geen BRCA-mutatie
risicofactoren zijn voor falen van PARPi-onderhoudstherapie na primaire behandeling
voor aOC.
