In preklinische modellen en klinische studies zijn aanwijzingen gezien voor synergie tussen lenalidomide en ibrutinib voor diffuus grootcellig B-cel lymfoom (DLBCL). De fase 2-studie Smart Start van MD Anderson Cancer Center (Houston TX) heeft de combinatie van rituximab, lenalidomide, en ibrutinib (RLI) voorafgaand aan chemotherapie voor nieuw-gediagnostiseerd DLBCL geëvalueerd. Dr. Jason Westin en collega’s publiceren de studie in het Journal of Clinical Oncology.¹

De studie includeerde patiënten met nieuw-gediagnostiseerd non-germinal center B-cell-like DLBCL. De patiënten kregen twee cycli RLI, waarna standaard-chemotherapie aan RLI werd toegevoegd voor zes additionele cycli. Primaire eindpunten waren overall response rate na twee cycli alleen RLI, en complete response rate na voltooiing van RLI met chemotherapie.

De mediane leeftijd van de 60 geïncludeerde patiënten was 63,5 jaar (range 29-83; 28% 70 jaar of ouder). Volgens de revised international prognostic index had 42% van de patiënten hoog-risico ziekte; en 62% waren double expressors van MYC- en BCL2-eiwit. De ORR na twee cycli RLI was 86,2% en de CRR na voltooiing van RLI plus chemotherapie was 94,5%. De mediane follow-up was 31 maanden. De twee-jaars PFS- en OS-percentages waren 91,3% respectievelijk 96,6%.

De onderzoekers concluderen dat RLI een hoge ORR induceerde en RLI plus chemotherapie resulteerde in duurzame respons.

RLI induced a high ORR, and RLI with chemotherapy resulted in durable responses.
