Er is geen
duidelijkheid over de optimale behandelingsstrategie voor geresecteerd stadium
I large cell neuroendocrine carcinoma of
the lung (LCNEC). Dr. Janakiraman Subramanian (University of Missouri,
Kansas City) en collega’s hebben een analyse uitgevoerd van de impact van
systemische chemotherapie na chirurgische resectie voor stadium I LCNEC. Ze
publiceren de analyse online in het Journal of Thoracic
Oncology.1
In de National Cancer Database identificeerden
de onderzoekers 1232 patiënten die tussen begin 2004 en eind 2013 chirurgische resectie
ondergingen voor stadium I LCNEC. Van deze patiënten ondergingen er 957 alleen
resectie (77,7%) en kregen 275 na de resectie chemotherapie (22,3%). Het
primaire eindpunt van de analyse was overall
survival. De vijf-jaars OS was significant beter in de groep patiënten die
chemotherapie kreeg (64,5% versus 48,4%; p<0,001). Ook in multivariate analyse
was systemische chemotherapie na de resectie geassocieerd met significant
betere OS (HR 0,54; p<0,001). Het overlevingsvoordeel van systemische
chemotherapie werd bevestigd in propensity-gematchte analyse.
De
onderzoekers concluderen dat systemische chemotherapie na resectie voor stadium
I LCNEC resulteert in verbetering van OS.
