Voor de
meeste patiënten met progressie van niet-kleincellig longcarcinoom op eerste-
of tweedelijns immuuntherapie is salvage chemotherapie de enige potentieel werkzame
optie. De retrospectieve multicenter CLARITY-studie in Italië heeft de
werkzaamheid van salvage chemotherapy
after immunotherapy (SCAI) voor gevorderd NSCLC geïnventariseerd. Dr. Melissa Bersanelli (Universiteitsziekenhuis
Parma) en collega’s publiceren de studie in Lung Cancer.1
De studie
includeerde 342 patiënten die tussen november 2013 en augustus van 2019 in 20
Italiaanse centra SCAI kregen. Het primaire eindpunt van de studie was overall survival, gerekend vanaf de
start van SCAI. De mediane OS was 6,8 maanden (95%-bti 5,5-8,1). De mediane
progressievrije overleving was 4,1 maanden (95%-bti 3,4-4,8) en de objective response rate was 22,8%. De onderzoekers
construeerden een ‘post-checkpoint inhibition
score’ die op basis van geslacht, ECOG performance score, en ziektecontrole
met voorafgaande immuuntherapie de patiënten kon onderverdelen in drie prognostische
groepen; de Harrell’s C-index was 0,65 (95%-bti 0,59-0,71).
De
onderzoekers concluderen dat de studie steun geeft aan de hypothese dat
voorafgaande immuuntherapie de gevoeligheid van NSCLC voor chemotherapie
verhoogt. De post-CKI score kan wellicht bijdragen aan op individuele patiënten
toegesneden behandeling na falen van immuuntherapie.
