Sinds begin
2007 hebben in het ziekenhuis van het College of Medicine van de University of
Florida in Jacksonville patiënten definitieve of adjuvante external-beam
protontherapie gekregen voor chondrosarcoom van de schedelbasis. Dr. Adam
Holtzman en collega’s publiceren uitkomsten van de behandeling online in het Journal of Neuro-Oncology.1 De studie includeerde 43
patiënten met een mediane leeftijd van 49 jaar (range 23 tot 80 jaar). Negentien
patiënten (44%) hadden graad 1 tumoren, tweeëntwintig patiënten (51%) graad 2,
en twee patiënten (5%) graad 3.
Zesendertig
van de patiënten (84%) ondergingen chirurgische resectie voorafgaand aan de
protontherapie. De mediane radiotherapiedosering was 73,8 Gy RBE (range
64,5-74,4). Er waren geen gevallen van graad 3 of hoger acute toxiciteit. De
mediane follow-up was 3,7 jaar (range 0,7 tot 10,1 jaar). Vier jaar na de RT
was de overall survival 95%, de
ziekte-specifieke overleving 100%, locale controle 89%, en de RT-gerelateerde
graad 3 toxiciteit-vrije overleving 95%.
De
onderzoekers concluderen dat hoge-dosering protontherapie alleen of na
chirurgische resectie voor chondrosarcoom van de schedelbasis een effectieve
behandeling is die resulteert in een hoog percentage patiënten met locale controle
en geen acute graad 3 of hoger RT-gerelateerde toxiciteit.
