Oudere
vrouwen zijn ondervertegenwoordigd in klinische studies van mammacarcinoom. De Cancer
and Leukemia Group B 49907 studie randomiseerde vrouwen in de leeftijd van
65 jaar en ouder met vroeg-stadium mammacarcinoom naar standaard adjuvante
chemotherapie (physician’s choice van
cyclofosfamide plus methotrexaat plus fluorouracil of cyclofosfamide plus
doxorubicine) of capecitabine. In 2009 is gepubliceerd dat na mediaan 2,4 jaar follow-up de recidiefvrije overleving en overall survival significant langer
waren in de standaard-chemotherapiegroep dan in de capecitabinegroep. Prof.
Hyman Muss (University of North Carolina, Chapel Hill) en collega’s publiceren nu online in het Journal of
Clinical Oncology een update van de resultaten van de studie na mediaan
11,4 jaar follow-up.1
De studie
includeerde 633 patiënten, die 1:1 werden gerandomiseerd naar
standaard-chemotherapie of capecitabine.
Na tien jaar was de RFS 56% in de standaard-chemotherapiegroep versus
50% in de capecitabinegroep (HR 0,80; p=0,03). Het RFS-verschil tussen beide
armen was statistisch significant onder patiënten met HR-negatieve ziekte (HR
0,66; p=0,02) maar niet onder patiënten met HR-positieve ziekte (HR 0,89;
p=0,43). De mammacarcinoom-specifieke overleving was 88% versus 82% (HR 0,62;
p=0,03). De overall survival was 62%
versus 56% (HR 0,84; p=0,16). Na mediaan 11,4 jaar follow-up waren 43,9% van de
patiënten overleden: 13,1% aan mammacarcinoom; 16,4% aan andere oorzaken; en
14,1% aan niet-bekende oorzaken (althans niet bekend bij de onderzoekers).
De
onderzoekers concluderen dat met langere follow-up de RFS significant beter
bleef met adjuvante standaard-chemotherapie dan met capecitabine, vooral in de
groep patiënten met HR-negatieve ziekte. Het aanvankelijk waargenomen
OS-voordeel van standaard-chemotherapie versus capecitabine is verdund door competing risks in deze oudere
populatie.

Summary: The
CALGB 49907 study compared standard adjuvant chemotherapy versus capecitabine
for early stage breast cancer in women 65 years of age or older. Ten-year follow-up of the study showed that the recurrence-free survival remained superior for standard
chemotherapy, especially in patiens with HR-negative disease. Competing risks
in this older population diluted overall survival benefit of the standard chemotherapy
over capecitabine.
