Perioperatieve chemotherapie met fluorouracil, leucovorine, oxaliplatine, en docetaxel (FLOT) is een standaard-optie voor patiënten met resectabel oesofagogastrisch adenocarcinoom (EGA). De Duitse multicenter fase 2-3 studie DANTE evalueert toevoegen van atezolizumab (anti-PD-L1) aan perioperatief FLOT voor deze patiënten. Prof. Sylvie Lorenzen (Technische Universität München) en collega’s publiceren resultaten van het fase 2-gedeelte van de studie in het Journal of Clinical Oncology.¹

De studie includeerde 295 patiënten met resectabel EGA (≥cT2 of cN+), onder wie 23 patiënten (8%) met microsatelliet-instabiele (MSI) tumoren en 171 patiënten (58%) met een PD-L1 combined positive score (CPS) 1 of hoger. De patiënten werden gerandomiseerd naar vier preoperatieve en vier postoperatieve cycli FLOT plus atezolizumab gevolgd door acht cycli atezolizumab onderhoudsbehandeling (arm A) of alleen FLOT (arm B). Er waren geen verschillen tussen de armen in chirurgische morbiditeit (45% van de patiënten in arm A versus 42% in arm B) en zestig-dagen mortaliteit (3% versus 2%). Downstaging was gunstiger in arm A dan in arm B: ypT0 23% versus 15% (p=0,044), ypT0-T2 61% versus 48% (p=0,015), ypN0 68% versus 54% (p=0,012). Histopathologisch compleet regressiepercentage was hoger in arm A dan in arm B (24% versus 15%; p=0,032); dit verschil was groter in de subpopulaties met PD-L1 CPS 10 of hoger (33% versus 12%) en met MSI-tumoren (63% versus 27%). R0-resectiepercentages waren hoog in beide armen (96% versus 95%). Incidentie en ernst van adverse events verschilden niet significant tussen de beide armen.

De onderzoekers concluderen dat toevoegen van atezolizumab aan perioperatief FLOT voor resectabel EGA veilig was en postoperatief stadium en histopathologische regressie verbeterde.
