Tumor Treating Fields (TTFields) zijn electrische velden die processen verstoren die van belang zijn voor overleving van maligne cellen. De multinationale fase 3-studie LUNAR evalueerde toevoegen van TTFields aan standaard systemische behandeling voor metastatisch niet-kleincellig longcarcinoom (mNSCLC) na progressie op of na chemotherapie. Dr. Ticiana Leal (Emory University, Atlanta GA) en collega’s publiceren de studie in The Lancet Oncology.¹

LUNAR werd uitgevoerd in 130 centra in negentien landen. De studie includeerde 276 patiënten in de leeftijd van 22 jaar of ouder, met progressief mNSCLC na platina-gebseerde chemotherapie, en een ECOG performance status 2 of beter. De patiënten werden 1:1 gerandomiseerd naar TTFields plus standaard systemische therapie (behandelaars keus uit nivolumab, pembrolizumab, atezolizumab; n=137) of alleen systemische therapie (‘controlegroep’; n=139). TTFields (150 kHz) werd continu toegepast op de thorax, met advies toepassing gedurende tenminste 18 uur per dag. Het primaire eindpunt was overall survival.

De mediane follow-up was 10,6 maanden in de TTFieldsgroep en 9,5 maanden in de controlegroep. De OS was significant langer met TTField plus standaardtherapie dan met alleen standaardtherapie (mediaan 13,2 maanden versus 9,9 maanden; HR 0,74; p=0,035). Ernstige adverse events of any cause werden gerapporteerd voor 53% van de patiënten in de TTFieldsgroep en 38% van de controlegroep. De TTFields-gerelateerde AEs waren voornamelijk graad 1 of 2 aandoeningen van huid of subcutaan weefsel. In de TTFieldsgroep werden geen graad 5 TRAEs gezien, vergeleken met drie in de controlegroep.

De onderzoekers concluderen dat onder patiënten met progressief mNSCLC na platina-gebaseerde chemotherapie, toevoegen van TTFields aan standaard systemische therapie resulteerde in verlenging van de OS zonder verergering van de systemische toxiciteiten.
