Er is
behoefte aan nieuwe strategieën voor de preventie van graft-versus-host disease (GVHD) in patiënten die transplantatie
van niet-verwante donoren ondergaan voor hematologische maligiteiten. Een fase
3-studie in Canada en Australië heeft de waarde onderzocht van toevoeging van
antithymocytenglobuline (ATG) aan standaard GVHD-profylaxe. In 2016 is gepubliceerd dat toevoeging van ATG resulteerde in een hoger percentage
patiënten die na twaalf maanden vrij waren van immuunsuppressieve behandeling
(primair eindpunt van de studie). Prof. Irwin Walker (Juravinski Hospital,
Hamilton ON) en collega’s publiceren nu twee-jaars resultaten van de studie online in The Lancet Haematology.1
De studie
werd uitgevoerd in tien centra in Canada en één centrum in Australië. De studie
includeerde patiënten met leukemie, MDS, of lymfoom, leeftijd van zestien tot
en met zeventig jaar, en een Karnofsky score van tenminste 60, die
myeloablatieve of reduced intensity conditioning
transplantatie van een niet-verwante donor ondergingen. Ze werden
gerandomiseerd naar ATG 4,5 mg/kg plus standaard GVHD-profylaxe (cyclosporine
of tacrolimus plus methotrexaat of mycofenolaat; n=99) of alleen standaard
GVHD-profylaxe (n=97).
Twee jaar na
de transplantatie was 38% van de patiënten in de ATG plus standaard
GVHD-profylaxegroep versus 19% van de patiënten in de standaard
GVHD-profylaxegroep vrij van immuunsuppressieve therapie (OR 3,49; p=0,0016).
De cumulatieve incidentie van relapse was 16,3% versus 17,5% (p=0,73), en de
nonrelapse mortaliteit was 21,2% versus 31,3% (p=0,15). De cumulatieve
incidentie van GVHD was 26,3% versus 41,3% (p=0,032). De overall survival was 70,6% versus 53,3% (HR 0,56; p=0,017); in de abstract van de publicatie zijn deze twee percentages abusievelijk verwisseld. In de
standaard GVHD-profylaxegroep waren depressiesymptomen meer prominent. Ernstige
adverse events (graad 4 of 5) werden
gezien in 38% van de patiënten in de ATG plus standaard GVHD-profylaxegroep
versus 51% in de standaard GVHD-profylaxegroep.
De
onderzoekers concluderen dat toevoegen van ATG aan standaard GVHD-profylaxe
resulteerde in significant betere uitkomsten van unrelated donor
transplantation onder patiënten met hematologische maligniteiten.

Summary: A phase 3 study in Canada and
Australia found that addition of anti-thymocyte globulin to standard graft-versus-host
disease prophylaxis in patients with hematologic malignancies undergoing
transplantation from unrelated donors provided clinically meaningful benefits;
including longer overall survival, a decrease in use of immunosuppressive
therapy, chronic GVHD and its symptoms, and depressive symptoms.
