Patiënten
met gevorderd of recidiverend carcinosarcoom van de uterus (A/R UCS) hebben
weinig behandelopties. De multicenter STATICE-studie in Japan evalueerde
trastuzumab deruxtecan (T-DXd; een op HER2 gericht antibody-drug conjugate) voor A/R UCS met HER2-expressie. Dr. Kan
Yonemori (National Cancer Center Hospital, Tokio) en collega’s publiceren de studie in het Journal of Clinical
Oncology.1
De studie
includeerde patiënten in twee groepen, met hoge (IHC 2 of hoger; n=22) of lage
(IHC 1) expressie van HER2. De patiënten kregen intraveneus T-DXd 6,4 of 5,4
mg/kg iedere drie weken tot progressie of niet-acceptabele toxiciteit. Het
primaire eindpunt was centraal-beoordeelde objective
response rate in de groep met hoge HER2-expressie. De figuur laat zien dat T-DXd activiteit had in beide groepen. De mediane PFS
en OS waren 6,2 respectievelijk 13,3 maanden in de groep met hoge HER2-expressie
en 6,7 maanden respectievelijk niet bereikt in de groep met lage
HER2-expressie. Het veiligheidsprofiel van T-DXd was consistent met eerdere
rapporten. Toxiciteiten waren manageable.
De
onderzoekers concluderen dat T-DXd de eerste anti-HER2 therapie is met
klinische activiteit voor A/R UCS.

multicenter STATICE trial in Japan
evaluated the HER2-targeting antibody-drug conjugate trastuzumab deruxtecan
(T-DXd) for HER2-expressing advanced or recurrent uterine carcinosarcoma.
Patients were recruted in groups with high (IHC score ≥ 2) or low (IHC score=1)
HER2-expressing UCS. T-DXd had efficacy in both groups. Toxicities were
manageable with appropriate monitoring and treatment.
