Gelet op het
toenemend aantal oudere of comorbide patiënten met AML of MDS is er behoefte
aan verdere verbetering van conditioneringsregimes voor allogene stamceltransplantatie
(alloHSCT). Een multinationale non-inferioriteits fase 3-studie heeft treosulfan-fludarabine
vergeleken met reduced-intensity
busulfan-fludarabine voor patiënten die niet geschikt werden geacht voor
standaard-conditioneringsregimes. Prof. Dietrich Wilhelm Beelen (Universiteitsziekenhuis
Essen, Duitsland) en collega’s publiceren de studie online in The Lancet Haematology.1
De studie
werd uitgevoerd in 31 centra in Duitsland, Frankrijk, Hongarije, Italië en
Polen. De patiënten waren geïndiceerd voor alloHSCT voor AML in hematologische
remissie (blast counts <5% in beenmerg) of MDS (< 20%). Ze waren achttien
tot en met zeventig jaar oud, hadden een Karnofsky index 60% of hoger, en waren
geen kandidaat voor standaard-conditionering vanwege leeftijd vijftig jaar of
ouder, en/of een HSCT-specifieke comorbiditeitenindex 2 of hoger. Ze werden
gerandomiseerd naar treosulfan-fludarabine (n=240) of reduced-intensity busulfan-fludarabine (n=221) conditionering,
gevolgd door alloHSCT in alle patiënten behalve één in de busulfan-groep. Het
primaire eindpunt was gebeurtenisvrije overleving twee jaar na de HSCT, met een
non-inferioriteitsmarge HR 1,3.
De studie
werd voortijdig gestopt toen bij een interimanalyse superioriteit van
treosulfan-fludarabine werd vastgesteld. De mediane follow-up was 15,4 maanden
in de treosulfangroep en 17,4 maanden in de busulfangroep. De twee-jaars EFS
was 64,0% in de treosulfangroep versus 50,4$ in de busulfangroep (HR 0,65;
p<0,0001 voor niet-inferioiriteit; p=0,0051 voor superioriteit). Er waren
geen significante verschillen tussen beide groepen in het optreden van graad 3
of hoger adverse events, met
uitzondering van minder gastroïntestinale klachten met treosulfan dan met
busulfan (11% versus 16%).
De
onderzoekers concluderen dat treosulfan-fludarabine als conditioneringsregime
voor alloHSCT in oudere of comorbide patiënten met AML of MDS niet-inferieur was
aan busulfan-fludarabine, en resulteerde in betere twee-jaars EFS.
