Chemotherapie-geïnduceerde
myelosuppressie kan resulteren in aanzienlijke bijwerkingen. Trilaciclib is een
intraveneus toegediende CDK4/6-remmer die hematopoïetische stamcellen en
progenitorcellen tegen chemotherapie-geïnduceerde schade kan beschermen. Een
gepoolde analyse van drie fase 2-studies met extensief-stadium kleincellig
longcarcinoom (ES-SCLC)-patiënten heeft myeloprotectie door trilaciclib

De drie
studies behandelden ES-SCLC patiënten met eerstelijns etoposide-carboplatine,
eerstelijns etoposide-carboplatine-atezolizumab, en tweede- of derdelijns
topotecan. De patiënten werden gerandomiseerd naar trilaciclib (n=123) of
placebo (n=119) voorafgaand aan de chemotherapie. De trilaciclibgroep had
significant lagere scores voor de meeste symptomen van chemotherapie-geïnduceerde
myelosuppressie, resulterend in lagere noodzaak van interventies, minder
hospitalisaties, en in verbetering in domeinen van gezondheids-gerelateerde
kwaliteit van leven, waaronder vermoeidheid, fysiek welzijn, en functioneren.
Trilaciclib had geen impact op de antitumorwerkzaamheid van de chemotherapie.
De
onderzoekers concluderen dat trilaciclib voorafgaand aan chemotherapie voor
ES-SCLC resulteerde in klinisch relevante afname van chemotherapie-geïnduceerde
myelosuppressie en verbetering van HRQoL, zonder impact op de effectiviteit van
de chemotherapie.

Summary: Pooled analysis of three randomized
phase 2 studies found that administering trilaciclib prior to chemotherapy for
ES-SCLC resulted in clinically meaningful reductions in chemotherapy-induced
myelosuppression, and improved patient HRQoL, with no impact on the antitumor
efficacy of the chemotherapy.
