Er is
behoefte aan biomarkers voor het geleiden van escalatie of deëscalatie van
systemtische therapie voor vroeg-stadium HER2-positief mammacarcinoom. De CelTIL-score is een nieuwe biomarker die gebaseerd is op tumorcellulariteit en
tumorinfiltrerende lymfocyten. De score wordt bepaald na twee weken anti-HER2
therapie. Een analyse in het cohort van de fase 3-studie NeoALTTO heeft de
prognostische waarde van CelTIL geëvalueerd.

De analyse
includeerde 196 patiënten die lapatinib, trastuzumab, of lapatinib plus
trastuzumab kregen voor vroeg-stadium HER2-positief mammacarcinoom. Op dag 15
van de behandeling werd de CelTIL-score bepaald. Op basis van een
vooraf-gespecificeerde afsnijwaarde werden twee groepen patiënten
onderscheiden. De groep met hoge CelTIL-score had vergeleken met de CelTIL-lage
groep betere vijf-jaars gebeurtenisvrije overleving (76,4% versus 59,7%; HR
0,40; 95%-bti 0,17-0,94) en betere vijf-jaars overall survival (86,4% versus 73,5%; HR 0,43; 95%-bti 0,20-,0,92).
Statistische significantie bleef behouden na correctie voor baseline TILs,
hormoonreceptorstatus, pre-treatment tumorgrootte en nodale status, type
chirurgie, behandelarm, en pathologisch complete respons.
De
onderzoekers concluderen dat CelTIL-score een vroege indicatie gaf van
werkzaamheid van behandeling voor HER2-positief mammacarcinoom.

Summary: In
early-stage HER2-positive breast cancer, biomarkers are needed that guide
escalation and/or de-escalation of systemic therapy. CelTIL score is a novel

cellularity. The score is determined at day 15 of anti-HER2 therapy. Among
patients in the NeoALTTO phase 3 trial, CelTIL-high versus CelTIL-low score was associated with 5-year EFS of 76.4%
