De
standaardbehandeling voor HPV-positief squameus celcarcinoom van de orofarynx
(OSCC) is radiotherapie plus cisplatine. Twee studies die online zijn
gepubliceerd in The Lancet
onderzochten de mogelijkheid cisplatine in deze behandeling te vervangen door
cetuximab om de toxiciteit van de behandeling te verminderen.1,2RTOG 1016 werd uitgevoerd in 182 centra in Canada en de Verenigde Staten. De-ESCALaTE HPV werd uitgevoerd in 32 centra in Ierland, Nederland, en het
Verenigd Koninkrijk.

randomiseerde 425 patiënten naar RT plus cetuximab en 424 naar RT plus
cisplatine. De follow-up was mediaan 4,5 jaar. De vijf-jaars overall survival
was 77,9% in de cetuximabgroep versus 84,6% in de cisplatinegroep (HR 1,45;
p=0,0163). De vijf-jaars progressievrije overleving was 67,3% in de
cetuximabgroep versus 78,4% in de cisplatinegroep (HR 1,72; p=0,0002). Vijf-jaars
cumulatief locoregionaal falen was 17,3% in de cetuximabgroep versus 9,9% in de
cisplatinegroep (HR 2,05; 95%-bti 1,35-3,10). Er waren voor acute en late
matige tot ernstige toxiciteiten geen significante verschillen tussen beide
groepen.
De-ESCALaTE
HPV randomiseerde168 patiënten naar RT plus cetuximab en 166 naar RT plus
cisplatine. Na 24 maanden follow-up was er geen verschil tussen beide groepen
in graad 3 tot 5 toxiciteit (p=0,98). De figuur laat zien dat de overall
survival en recidiefvrije overleving significant slechter waren in de
cetuximab-groep, terwijl er geen belangrijke verschillen waren in algemene
gezondheidsstatus.
De
onderzoekers van beide studies concluderen dat radiotherapie plus cisplatine de
standaardbehandeling dient te blijven voor HPV-positief OSCC in patiënten die
cisplatine kunnen verdragen.

Summary: Standard
of care for HPV-positive OSCC is radiotherapy plus cisplatin. Two studies
published online in The Lancet looked at the possibility of replacing cisplatin
with cetuximab. RTOG 1016 found that patients in
the RT+cetuximab arm had inferior OS and PFS compared with patients in the
RT+cisplatin arm. De-ESCALaTE HPV found that
RT+cetuximab showed no benefit in terms of reduced toxicity, but instead showed
significant detriment in terms of
tumor control.
