kunnen resulteren in respons van verscheidene typen maligniteiten, maar zijn
ook geassocieerd met immuun-gerelateerde adverse
events, waaronder cutane bijwerkingen (ircAEs). Een studie van patiënten
van drie tertiaire oncodermatologiecentra heeft de uitkomsten geïnventariseerd
van behandeling van deze ircAEs. Dr. Mario Lacouture (Memorial Sloan Kettering Cancer Center, New York)
en collega’s publiceren de studie online in het Journal of Clinical Oncology.1
De studie
includeerde 285 patiënten (mediane leeftijd 65 jaar; range 17-89) met tezamen
427 ircAEs: pruritus (32% van de ircAEs), maculopapulaire rash (28%),
psoriasiforme rash (5%), en overige (34%). Er was een associatie tussen type
ICI en fenotype van de ircAE (p=0,007); maculopapulaire rash was predominant in
patiënten die combinatietherapie kregen. De ernst van de ircAEs werd significant
verlaagd door dermatologische interventie (gemiddelde CTCAE-graad 1,74 versus
0,71; p<0,001), waaronder topische corticosteroïden, orale antipruritica, en
systemische immuunmodulatoren.
Onder de 88
ircAEs (20%) die behandeld werden met systemische immuunmodulatoren waren er 22
(25%) die persisteerden of slechter werden. Zeven patiënten met
corticosteroïden-refractaire ircAEs hadden baat bij gerichte biologische
immuunmodulerende therapie (rituximab en dupilimumab). Serum IL-6 was verhoogd
in 34 van 65 patiënten; graad 3 of hoger ircAEs was geassocieerd met verhoogd
gehalte eosinofielen (OR 4,1; 95%-bti 1,3-13,4) en IL-10 (OR 23,8; 95%-bti
2,1-262,5). De ircAE-graad was geassocieerd (p=0,043) met het gemiddelde
serumgehalte van immunoglobuline E (graad 3: 1093 kU/l; graad 2: 245 kU/l; en
graad 1: 112 kU/l).
De
onderzoekers concluderen dat de meeste ircAEs respons hadden op symptoom- en
fenotype-gerichte behandelingen, terwijl biologische therapie werkzaam was voor
corticosteroïden-refractaire ziekte. Verhoogde eosinofielen, IL-6, IL-10, en
immunoglobuline E waren geassocieerd met ircAEs.

patients with immune-related cutaneous adverse events (ircAEs) from checkpoint
inhibitors at three tertiary care hospitals found that most ircAEs responded to
symptom- and phenotype-directed dermatologic therapies, whereas biologic
therapies were effective in patients with corticosteroid-refrractory disease.
Increased levels of eosinphils, IL-6, IL-10 and immunoglobulin E were associated with
ircAEs.
