Volwassen patiënten met recidiverend of refractair B-precursor acute lymfoblastische leukemie (R/R BCP-ALL) hebben een zeer slechte prognose. Het monoklonaal antilichaam blinatumomab is een bispecifieke T-cell engager met activiteit als monotherapie voor R/R BCP-ALL. Een pilot study van het Centre Hospitalier Universitaire de Bordeaux (Frankrijk) heeft de combinatie van blinatumomab met intensieve VANDA-chemotherapie (etoposide, cytarabine, mitoxantron, dexamethason, en asparaginase) voor R/R BCP-ALL geëvalueerd. Prof. Arnaud Pigneux en collega’s publiceren de studie in het British Journal of Haematology.¹



De studie includeerde zeventien patiënten met Philadelphia-chromosoom negatief BCP-ALL in eerste relapse. Salvage met de combinatie VANDA-blinatumomab resulteerde in complete remissie in veertien patiënten (82%) en de mogelijkheid tot transplantatie in elf patiënten (65%). Het één-jaars leukemievrije-overlevingspercentage was 58,8% onder alle patiënten en 90,9% onder de patiënten die transplantatie hadden ondergaan.

De onderzoekers concluderen dat het VANDA-blinatumumob salvage regime voor Ph-negatief BCP-ALL in eerste relapse resulteerde in hoge percentages patiënten met complete remissie en transplantatie.

Summary: A pilot study at the University Hospital of Bordeaux (France) found that blinatumomab combined with intensive VANDA chemotherapy as salvage for Philadelphia chromosome-negative B-precursor ALL in first relapse resulted in a very high rate of complete remission and hematopoietic stem-cell transplatation.
