Er is geen duidelijkheid over de werkzaamheid van PARP-remmers in combinatie met chemotherapie gevolgd door onderhoudstherapie voor niet-eerder behandeld ovariumcarcinoom. De multinationale fase 3-studie VELIA/GOG-3005 onderzocht de werkzaamheid en veiligheid van veliparib toegevoegd aan eerstelijns carboplatine plus paclitaxel chemotherapie en als onderhoudstherapie voor hooggradig ovariumcarcinoom. De studie is gisteren gepresenteerd op de Annual Meeting van ESMO in Barcelona. Dr. Robert Coleman (MD Anderson Cancer Center, Houston TX) en collega’s publiceren de studie ook online in The New England Journal of Medicine.¹

De studie includeerde 1140 patiënten met niet-eerder behandeld stadium III of IV hooggradig sereus ovariumcarcinoom. Ze werden 1:1:1 gerandomiseerd naar chemotherapie plus placebo inductie gevolgd door placebo-onderhoud (controlegroep), chemotherapie plus veliparib inductie gevolgd door placebo-onderhoud (veliparib-inductiegroep), en chemotherapie plus veliparib inductie gevolgd door veliparib onderhoud (veliparib throughout groep). De inductie bestond uit zes cycli, en de onderhoudstherapie uit nog dertig cycli. Cytoreductieve chirurgie vond plaats voor aanvang of na drie cycli. Het primaire eindpunt van de studie was lokaal-beoordeelde progressievrije overleving in de veliparib throughout groep vergeleken met de controlegroep, achtereenvolgens bepaald in de groep patiënten met BRCA-mutatie, in de groep patiënten met homologe recombinatiedeficiëntie (HRD), en in gehele ITT-populatie.

In het BRCA-mutatiecohort was de mediane PFS 34,7 maanden in de veliparib throughout arm versus 22,0 maanden in de controlegroep (HR 0,44; p<0,001). In het HRD-cohort was de mediane PFS in de twee groepen 31,9 versus 20,5 maanden (HR 0,57; p<0,001) en in de ITT-populatie was de mediane PFS in de twee groepen 23,5 maanden versus 17,3 maanden (HR 0,68; p<0,001). De combinatie chemotherapie plus veliparib resulteerde in verhoogde incidentie van anemie en trombocytopenie, en van misselijkheid en vermoeidheid.

De onderzoekers concluderen dat de combinatie van carboplatine, paclitaxel, en veliparib inductie gevolgd door veliparib onderhoud resulteerde in significant langere PFS dan alleen chemotherapie inductie. De onafhankelijke waarde van toevoeging van veliparib alleen tijdens inductie was minder duidelijk.

Summary: The multinational phase 3 study VELIA/GOG-3005 evaluated the PARP inhibitor veliparib with first-line chemotherapy and as maintenance therapy for ovarian cancer. The study found that a regimen of carboplatin, paclitaxel, and veliparib induction therapy followed by veliparib maintenance therapy led to significantly longer progression-free survival than carboplatin plus paclitaxel induction therapy alone. The independent value of adding veliparib during induction therapy without veliparib maintenance was less clear.
