De Enthusiasm Hypothesis, in 1993 gepubliceerd door dr. Mark Chassin, verklaart geografische variatie in gebruik
van specifieke behandelingen uit lokale aanwezigheid van behandelaars met voorkeur
voor de behandeling in kwestie (‘enthusiasts’).
Prof. Stephen Hall (Queen’s University, Kingston ON) en collega’s hebben
onderzocht of de hypothese van toepassing kan zijn op de variatie in
chirurgische behandeling voor gedifferentieerd schildkliercarcinoom (DTC) in
Ontario. Ze publiceren hun studie online in JAMA Otolaryngology – Head & Neck
Surgery.1
De
bevolkings-gebaseerde studie includeerde 28.754 volwassen patiënten (78,6%
vrouwen; gemiddelde leeftijd 49 ± 14 jaar) die tussen begin 2000 en eind 2015
een initiële therapeutische schildklier-chirurgische procedure ondergingen. De
analyse onderscheidde totale thyreoidectomie versus minder-dan-totale
thyreoidectomie. De procedures werden uitgevoerd door 343 chirurgen, van wie in
de zestien jaar van de studie 48,1% minder dan tien en 9,3% meer dan 150 totale
thyreoidectomieprocedures uitvoerden. Gebruik van totale thyreoidectomie liep
sterk uiteen over de veertien geografische regio’s van Ontario, van 45,8% tot
77,1% van alle DTC-patiënten binnen een regio. Meer dan 90% van de overall
variatie kon worden verklaard uit de praktijken van de hoog-volume chirurgen in
regio’s met de hoogste percentages DTC-diagnosen.
De
onderzoekers concluderen dat de resultaten van de studie consistent zijn met de
Enthusiasm Hypothesis.

Summary: A population-based study in Ontario found that the use of total thyroidectomy for differentiated thyroid cancer
(2000-2015) varied widely across geographic regions, from 45.8% to 77.1% of all
cases. More than 90% of the overall variation in practice (total versus less
than total thyroidectomy) could be explained by the practices of the
high-volume surgeons in regions with the highest rates of thyroid cancer
diagnosis. These findings were consistent with the Enthusiasm Hypothesis.
