Voorspellers van progressie tijdens inductiechemotherapie en mogelijkheid van chemotherapievrij interval in mCRC


Prof. Thomas AparicioDe prognose van patiënten met metastatisch colorectaalcarcinoom (mCRC) is sterk verbeterd dor gebruik van verscheidene achtereenvolgende chemotherapeutica. Gelet op de relatief lange overleving, die onder behandeling twee tot drie jaar kan duren, is er interesse voor de mogelijkheid van chemotherapievrije intervallen (CFIs) om te zware belasting en toxiciteit door de behandeling te vermijden. Een analyse in het cohort van de Franse PRODIGE 9 studie heeft gezocht naar voorspellers van progressie tijdens inductiechemotherapie en van de mogelijkheid van CFI in mCRC. Prof. Thomas Aparicio (Université de Paris) en collega’s publiceren de analyse online in het British Journal of Cancer.1

De figuur toont de flowchart van de studie. In multivariate analyse waren baseline leukocyten-niveaus > 1010/l (OR 1,98; p=0,04) en stabiele of toenemende CEA-niveaus na twee maanden (OR 3,61; p=0,01) onafhankelijke voorspellers van progressie tijdens de inductiechemotherapie. Mannelijk geslacht (OR 1,725; p=0,09), geen tumorrespons bij eerste evaluatie (OR 1,90; p=0,07), en aanwezigheid van BRAF-V600E mutatie (OR 4,59; p=0,058) waren significant geassocieerd met kort CFI.

De onderzoekers concluderen dat hoge baseline leukocytenniveaus en gebrek aan respons patiënten zouden kunnen identificeren met hoog risico van vroege progressie tijdens inductiechemotherapie en die profijt kunnen hebben van vroege intensivering van de chemotherapie. Mannelijk geslacht, geen respons bij eerste evaluatie, en aanwezigheid van BRAF-mutatie kan aanleiding zijn tot overwegen van onderhoudstherapie na inductiechemotherapie.

Summary: An analysis in the cohort of the French PRODIGE 9 study in mCRC found that high baseline leukocyte count and lack of CEA level decrease at first evaluation were associated with early progression during induction chemotherapy, and could identify patients who benefit from early chemotherapy intensification. Male gender, no tumor response at first evaluation, and BRAF mutation were associated with a short chemotherapy-free interval, and could identify patients who benefit from maintenance chemotherapy after induction chemotherapy.
