Er is geen
duidelijkheid over de waarde van toevoeging van arseentrioxide (ATO) aan
frontlijn-behandeling voor acute promyelocytaire leukemie (APL) in patiënten in
de leeftijd van 70 jaar en ouder. Een multinationale studie heeft deze waarde
onderzocht. Dr. Sabine Kayser (Universiteitsziekenhuis Leipzig) en collega’s
publiceren de studie online in Leukemia.1
De studie
includeerde 433 patiënten. De mediane leeftijd was 73,4 jaar. De patiënten
kregen ATO plus all-trans retinoïnezuur (ATO/ATRA; n=26), standaard
chemotherapie plus ATRA en ATO tijdens consolidering (CTX/ATRA/ATO; n=148), of
CTX/ATRA (n=259). Complete remissie werd gezien in 92% van de patiënten met
ATO/ATRA en 82% van de patiënten met CTX/ATRA, met induction death rates van 8%
respectievelijk 18%.
De mediane follow-up was 4,8 jaar. Voor
de analyse van de uitkomsten na remissie werden de ATO/ATRA en CTX/ATRA/ATO
groepen gecombineerd (ATO/ATRA ± CTX). De cumulatieve incidentie van relapse
was significant lager na ATO/ATRA ± CTX dan na CTX/ATRA (p<0,001). De
overall survival van patiënten in eerste complete remissie was niet
verschillend tussen ATO/ATRA ± CTX versus CTX/ATRA (p=0,20). Hoge
leukocytengetallen (≥ 10 x 109/l) bij diagnose waren geassocieerd
met hogere cumulatieve incidentie van relapse in de CTX/ATRA groep (p<0,001)
maar niet in de ATO/ATRA ± CTX groep (p=0,48).
De
onderzoekers concluderen dat toevoegen van ATO aan ATRA of CTX voor APL in
oudere patiënten feasible en effectief
was voor remissie-inductie en consolidatie, met name in patiënten met hoge WBC
bij diagnose.
